1. Your needs
From the moment you call, we begin considering you and your loved one’s specific needs. On the initial call, we ask questions to form a basic needs assessment.
We offer a free, comprehensive, no obligation assessment of your needs. Our care professional will come to your home to discuss your situation with you and your family, listening carefully to your needs, bringing our own expertise and answering all your questions. Assessments can also be carried out at other locations such as hospitals or respite care premises and will be arranged at a time of your choosing.
2. Individually tailored care plan
With Right at Home you will always know what services are being provided to your loved one. Following the initial meeting, we will create a tailored care plan to suit your specific individual needs and wishes. We will discuss every aspect of the plan with you and will consult medical professionals and other members of your support circle where necessary, in order to make sure our proposals both reflect your personal choices and that you receive the most appropriate forms of quality assistance. Our intention is to be as unobtrusive as possible, allowing you to determine your needs, whilst helping you to maintain an independent lifestyle.
3. Your choice of Carer
We will work closely with you to select and introduce a care team of your choice and agree visits at times to suit you. We take numerous factors into consideration: the services needed, your loved one’s interests and the personalities of both your loved one and the Carer. Where possible we organise introductions ahead of care starting and of course, if there are ever any concerns about a specific Carer, we are able to match you with someone you feel is more suitable.
4. Service inspection and follow up
We continually strive to be better, a culture driven from the top of the business and embraced by all, and visit all clients on a regular basis to ensure they receive the care and support agreed and that we surpass client expectations.
In addition to this our Tyneside office owner and managers make regular unannounced spot checks during visits, to ensure Carer are following the specifics of the Personal Care Plan, maintaining our high standards of care and responding immediately to any changes in your needs or circumstances
5. Your ongoing visits and updates
In addition to the quality measures outlined above, we will constantly review with you your individual care plan to reflect your changing circumstances with you and discuss and agree any amendments with you and your family. Our Carers keep extensive notes about the care they provide. Those notes are kept at yours or your loved one’s home and are available to read at any time. Our billing system also provides a detailed description of the services and care we have provided.
